    Welcome to CHS Athletics! Below are answers to Frequently Asked Questions.

    What are the costs for playing sports?

    Required costs are the ASB Card ($35 – one-time fee) and the Sports Participation Fee ($25 per sport), both of which need to be paid PRIOR to the first practice. Payments can be made to the ASB Activities Office by check or cash.

    How long are physicals good for?

    At CHS we accept physicals for one year. Athletes at this age are growing and changing rapidly, and a yearly physical for sports is in the best interest of the student athlete. If your athlete’s physical expires during their season, they will need to submit an updated one to the Athletic Office by the expiration date in order to remain eligible to participate.

    Can my student participate in practices without being cleared?

    No. Students must be cleared through the Athletic Office prior to their first practice. They cannot turn forms into their coach.

     How does a student get cleared to practice?

    An athlete is cleared to participate when they COMPLETE ALL of the following:

    1. Submit to the Athletic Office the Medical History/Physical form.
    2. Submit online the following forms on rankonesport.com: Activity Clearance, Safety Guidelines & Warning of Risk, Sudden Cardiac Arrest & Concussion, and Athletic/Activity Code
    3. Pay the Athletic Fees - $35 ASB Card and $25 Sport Participation
    4. Return any prior years/seasons books/uniforms/gear and pay all prior fines/fees/obligations.

     How do we access the Clearance forms on Rank One Sport?

    Visit www.rankonesport.com. Click on Fill Out Electronic Forms, select WA state, click on Centralia School District, then Electronic Participation Forms. Then complete the forms listed.

     If my student plays a sport each season, do they fill out the forms again for each one?

    No, as long as the Safety Guidelines and Warning of Risk is complete for each sport they are interested in participaing in, Clearance Forms need to be filled out only once per school year.

     What are the 2019-20 1st Practice Dates and deadlines to have Clearance Forms and payments submitted?

    Fall Sports – Friday, August 16th - 1st Practice August 21st (Football), August 26th (Remaining Fall Sports)

    Winter Sports – Wednesday, November 13th - 1st Practice November 18th 

    Spring Sports – Wednesday, February 26th - 1st Practice March 2nd
