REGISTRATION FORMS AND INFORMATION FOR GRADES K-6
Please note that response times may vary based on the date you submit your registration packet. Your child will not be placed on a class list until all required forms and documents have been received at their school.
Students in grades 4-6 attend classes at either Oakview or Washington elementary schools. If you are unsure which school boundary you reside in, please contact our transportation department at (360)330-7628 and they will be able to help you determine which school you must enroll your child in.
If you are registering a student who is brand-new to the Centralia School District, you will need to complete each of the forms below and return them to your child's school. Your child will not be placed on a class list until all of these forms and required documents are received. In addition, you will need to bring a copy of your child's birth certificate and proof of your address (utility bill, lease agreement, etc.). It is not necessary to submit a registration form if you have used the online registration process. Forms may also be submitted electronically during the Skyward Family Access online registration process.

How do I set my child up for transportation?
If your child will be riding the bus to and/or from school, you will need to fill out the Transportation Registration Form. Our transportation department will contact you before the start of the school year to give you pertinent busing information.
What about lunch and/or breakfast?
The Centralia School District currently participates in the USDA Community Eligibility Provision Program, which allows us to provide lunch and breakfast to our K-3 students at no charge. In order to ensure that the District continues to comply with requirements of other federal and state funding programs, parents will need to return this income survey to their child's school with their other registration forms. The information on these forms is confidential and is only used to determine Centralia School District eligibility for certain government-funded educational programs.
Online Registration Process
Once you have gathered and completed all of the required forms and documents, you are ready to begin the online registration process. You may attached scanned copies of all required forms and documents to the application, or you may deliver them to your student's school in person. Please note that school offices are not open during summer break and that students will not be placed in classes until all required documents have been received.
