Below are the forms required for an athlete to turn out for a sport at Centralia High School. All athletes must complete and return the 4 required Clearance Packet Forms to the Athletic Office prior to being cleared to turn out for practice. If an athlete is a Home School, Alternative School, or Running Start student, please also print and complete the additional form and attach to the Clearance Packet.
